Simultaneous Quantification of Areca Nut- and Tobacco-Specific Nitrosamines in Human Saliva by Liquid Chromatography-Tandem Mass Spectrometry

The concurrent use of areca nut (AN) and tobacco is widespread, with strong evidence indicating elevated cancer risk. Areca nut- and tobacco-specific nitrosamines (ASNAs and TSNAs) are recognized human carcinogens due to their genotoxicity following metabolic activation. ASNAs and TSNAs can form through the nitrosation of AN- and tobacco-derived alkaloids in the presence of nitrite, yet only a few specific compounds have been detected in the oral cavity. No analytical methods are available for their simultaneous determination.
This study develops a novel liquid chromatography-tandem mass spectrometry (LC-QqQ-MS) method to simultaneously quantify five ASNAs (N-Nitrosoguvacoline, NGCO; N-Nitrosoguvacine, NGCI; 3-(Methylnitrosamino) propionitrile, MNPN; N-Nitrosonipecotic acid, NNIP; 3-(Methylnitrosamino) propionaldehyde, MNPA) and five TSNAs (N-Nitrosonornicotine, NNN; N-Nitrosoanatabine, NAT; N-Nitrosoanabasin, NAB; 4-(Methylnitrosamino)-1-(3-pyridyl)-1-butanone, NNK; 4-(Methylnitrosamino)-4-(3-pyridyl)-1-butanol, NNAL) in saliva. Saliva samples from individuals who chewed AN and smoked were pretreated using solid-phase extraction. LC-QqQ-MS was operated in positive ion mode with multiple reaction monitoring. This study is the first to report ASNAs (NGCI, NGCO, and NNIP) and TSNAs (NNK, NNN, NAB, NAT, and NNAL) in saliva, providing direct evidence of AN and tobacco co-exposure at concentrations ranging from 0.005 to 132.7 ng/mL. These findings improve understanding of the carcinogenic potential of concurrent AN and tobacco use.
